Renewal of Current Credentialing Class of 2018
| If you are a current member of the Credentialing or Master credentialing class of 2018, your five (5) year Verification Renewal necessary to maintain your Credentialing/Master Credentialing status is due April 1, 2023. Forms can be obtained by logging into the Member’s Section of the CDT Website and using the above link. The 5 Year Verification AND the below Verification are both due. |
Annual Verification for Current Credentialing
ALL current Credentialed and Master Credentialed individuals must complete and return your Annual Verification of Good Standing form found HERE. NOTE: new submission date for these is April 1, 2023. If you are in the class of 2018 and submitting your 5 Year Verification, you are still required to submit this Annual Verification.
